Express Scripts

State Health Plan coverage includes prescription benefits at no extra cost. Prescription drugs are a major benefit to you and a major part of the cost of our self-insured health plan. Prescription drugs are only covered at network pharmacies. Express scripts is the pharmacy benefits manager.
Family Medical Leave Act (FMLA)

The Family Medical Leave Act (FMLA) of 1993 is a federal law designed to help employees balance their jobs and responsibilities for their own health and the health of their families. All paperwork MUST be completed if you are out of work for 4 or more consecutive days. You will need to turn in all forms to the District Office. If your doctor lists any restrictions, these MUST BE approved by the District Office BEFORE you are allowed to return to work.

Health Coaching

A health coach can help you with behavioral health issues, chronic conditions, healthy lifestyles and maternity support. Health coaching is available online or via phone at no cost to State Health Plan members.
Hello Heart

Hello Heart is an easy-to-use program offered to State Health Plan primary members (ages 18 and older) who have blood pressure readings of 130/80mmHg or above or who take blood pressure medication. The program helps you track, understand and manage your heart health.

Long Term Disability

Basic Long Term Disability and Supplemental Long Term Disability plans are administered by The Standard Insurance Company.
Long Term Disability Information
Basic Long Term Disability (BLTD)
All active, permanent, full-time employees are eligible for BLTD if they are covered under the State Health Plan.
BLTD is provided at no cost to the employee and the benefit waiting period is 90 days.
Monthly BLTD benefit is 62.5% of your pre-disability earnings, reduced by deductible income and the maximum benefit is $800 per month.
Supplemental Long Term Disability (SLTD)
All active, permanent, full-time employees are eligible for SLTD.
SLTD also has a benefit waiting period of 90 days.
Monthly SLTD benefit is 65% of your pre-disability earnings, reduced by deductible income and the maximum benefit is $8,000 per month.
Meru Health

Meru Health is an online health care provider that offers State Health Plan primary members an evidence-based program to reduce anxiety, stress, depression and burnout.
MetLife Advantages

If you currently have optional life insurance through the state, your benefits include access to MetLife Advantages. Some of these advantages are will preparation, funeral planning assistance, and grief counseling.
MoneyPlus

The Pretax Group Insurance Premium feature allows you to pay your State Health Plan premiums with money from your paycheck before taxes are withheld. With the feature, you benefit from having less taxable income in each of your paychecks, which means more spendable income.
Medical Spending Account (MSA):
Allows you to set aside money from your paycheck pretax to pay for out-of-pocket health care expenses, such as copayments and coinsurance.

Allows you to set aside money from your paycheck pretax to pay for child day care expenses and, in some cases, elder care expenses.

It's always better to address a health issue before it becomes too late. PEBA Perks are available at no cost to State Health Plan primary members at network providers and pharmacies.
Retirement

Strive

Strive is a program offered at no cost to State Health Plan primary members ages 16 and older. Sign up for Strive, a digital health platform that helps you build healthy habits. You’ll get daily cards customized to your goals to help you explore new ways to get healthier and earn rewards.
Telehealth

The State Health Plan offers two Telehealth options and both are an easy way to be treated for common conditions. The options are available 24/7, but are not meant to replace your primary care provider.
Tobacco Cessation

This benefit provides enrollment in the Quit For Life® program at no cost. It also includes a $0 copay for tobacco cessation drugs to eligible participants. An expert Quit Coach® will support you over the phone, online and via text. Your coach will help you follow a Quitting Plan that is customized to your needs.
Virta

Virta is a medically supervised, research-backed program that can help you reverse Type 2 diabetes. With Virta you can naturally lower and control your average blood sugar (HbA1c) while also reducing or eliminating diabetes medication and losing weight. Virta is offered at no cost to eligible State Health Plan primary members ages 18 and older.
